Little gish achievement not unlocked even tho i remember fighting gish and picking it up multiple times!

Seems like you got a glitch here. You can either try to simply unlock it on another save or use the Steam Achievement Editor.

The item collection screen and the secrets menu doesnt show it either and im sure i did it.

If it's not in your steam achievements, nor in your secret page nor in your item collection, you simply haven't unlocked it yet. Maybe you found it on a seeded run? Or maybe you found ball of tar.

Just try to find him again. He appears in about 1/3 runs for me in a boss room. Keep on looking to be certain it's a glitch

Im sure i unlocked it i picked it up multiple times. No it wasnt ball of tar.
