What's the best starting item for the lost when I have holy mantle

ive unlocked the holy mantle for the lost and it was a pain in the @$$ what's the best starter item that's not dead cat. Epic fetus? Brimstone?

Never take dr fetus if you are going for mega satan. Trust me, I died. Two favorites of mine are dead eye and deaths touch, but if you can get your hands on brimstone, that will give you the highest survivability

Tech X + Sacred Heart is far better than brimstone. Killed mega satan eaisily as the lost w/o damage.

Easy, gnawed leaf. Unless your going for boss rush/hush. Then all you need is some orbital or auto form of damage like BBF

Some of the best ones I've seen are Gnawed Leaf, Tech X, Brimstone, Daddy Long Legs, Toxic Shock, Shielded Tears, and Pyromaniac. Death's Touch, Halo of Flies, Big Fan, Sacrificial Dagger, and Mom's Knife arfe good too though not necessarily as good as some of the top tier items.

I'm also going to say starter deck. Sure tears/range up/pretty fly pills are awesome. But so is carrying 2x chariot cards for panic moments or a jera rune on the side. And blowing up mushrooms and getting things like joker cards to get extra devil deals on floors you wouldn't normally is great and can turn a failed run into a win.

Basically any time you'd see a pill, you'll instead see a card so extra joker/chariot/panic warp card chance. Also greater Diamonds/clubs/spades for consumables. If you see blank card it makes it more powerful if you ditch the D4.

If you have it unlocked you might get a chaos card for a difficult boss like hush or get out of jail to unlock mega satan.

I killed Mega Satan as The Lost with Libra + Sad Bombs + Dr. Fetus + Monstros Lung + Spoon Bender, the fight didn't last long at all
