Epic Fetus + Ipecac in the first floor

Epic Fetus can be found in the treasure room. Ipecac is just lying on the floor in the middle of some pots.
Epic Fetus already does a great amount of damage. Add to that Ipecac, which puts that damage to maximum.

"damage to maximum"? There is no max damage stat. You can always improve it further. Now given the astronomical bonus that ipecac gives, most normal damage ups won't increase it by proportionally much, but there are a few things you can get that add a good multiplier to your damage.

Also, the damage formula has a square root in it, so you can get diminishing returns by getting lots of damage ups, though multipliers are still always valuable, and the bonus from ipecac is outside the square root.

regardless, the damage is enough to one-shot most bosses.

Oh, I forgot to mention that the seed also has Mutant Spider at some point before Mom's Heart. The bosses that survive a hit surely won't survive four.

Ever get epic fetus plus brimstone? Each shot is like if you get tammy's head plus brimstone. Btw, the doctor's remote works exactly like an epic fetus shot. Get that, 9 volt and AAA battery, plus something that synergizes well with it, and you can instantly clear any boss just as well as if you had epic fetus.

hell yeah, I got Doctor's Remote as Azazel once. figured it'd be godly with Brimstone. too bad I was trying to go for Mega Satan so no devil deals for me.

epic fetus doesn't trigger sad bombs effect. However, if you have sad bombs plus ipecac, planting an actual bomb will send out a circle of ipecac shots, surely hitting you. In this run, Northerlion ends up with ipecac, cursed eye, my reflection, curse of the tower, sad bombs, and scatter bombs:
