Spingdown dice not spinning down to an item it had to spin down

Hey, I got a this spindown dice glitch 2 times this run, still on the run, got frustrated and confused is to why this glitch even accrued. Can someone help me with this question?

P.s. Is there a chance that some mods have anything to do with the glitch?

Edit: Got inside a boss challenge room, I saw tooth picks that could be spun down to Sacred heart, I got the sacred heart, thank god. But when I was in the treasure room and try to spindown a knife piece 1 to mega mush, already having rock bottom, it spun down to booster pack and just ignored mega mush all together

Could be caused by mods. Assuming there really is something unintended happening, of course.

Have you unlocked Mega Mush on that save? Locked items are skipped.

I didn't have mega mush unlocked. I saw the question mark next to some times, this was the problem. Thanks, you are very helpful:)
