placebo + hp up pill

just had a run as samson where i had placebo active item and an hp up pill. everytime i used placebo the pill would change from health up to health down or from health down to health up. is there an item that could cause this or is it a bug or something

All health down pills change to health up pills if you have 1 or less red heart containers. Once you get more than one, they revert back to health down. So you more than likely found a health down pill and discovered it when at 1 or less red hearts.

There's actually a funny bit of manipulating you can do with that as well. The Lost, due to having no health containers, cannot get either bad trips or health down pills, they will always be full healths and health ups respectively. This doesn't really matter for Lost specifically, but it does when you are in Lost form as someone else. Using stuff like white flames in Downpour, Soul of the Lost or Tainted Esau, you can turn a health down pill into a health up and also makes testing out said pills when unidentified more safe. It's these small little details that make this game so endlessly enjoyable to me.
