It's usually one, that comes with an outdated "items.xml"-file from an earlier game version, so if you have lots of mods, you can first try to narrow it down by disabling the ones, that add, change or remove items or their names and descriptions in-game. It can be caused by other types of mods as well, but that's where i'd start looking.
You can give yourself "The Black Bean" with the debug menu and take damage on purpose. If Isaac seems to never stop farting, you have a mod with an outdated items.xml.
