As for improvements to Dead Cat. I don't really think it needs one, but if it were to get one I'd still want it to function as 9 revives. Instead of setting your health to 1 red heart, it converts all hearts to soul hearts. You now take a full heart of damage regardless of chapter, but can revive with 2 soul hearts 9 times. This means you revive with essentially the same amount of health, but cannot lose deal chance.
Best use I've found is basically using it for nearly "free" devil deals. You'll lose a few of your lives but after about 5 or so of the right devil deals you'll be so OP (and probably have plenty of black/blue hearts) that the one heart thing won't matter anymore.
The game can suck because sometimes items can just screw you and ruin a run if you accidentally or just pick it up on a gamble. But vice versa it can propel your run into the stratosphere just as easily. Just how the game is.
Nine extra lives is nuts, really and already makes the item great. Exploit devil deals the whole run and not have to worry about dying since hey, you got lots of extra lives in case you do die to an enemy.
Dead Cat is so good it's a win condition for certain characters. Sure might not be good for Magdalene but even then a run isn't over if you get Dead Cat with her. You just not have to get hit and you still have those lives to do some cheese with.
your health is like blood coins. If you can avoid taking damage, that is. Dead cat, if taking the item is properly planned for and timed, allows you to get absurdly powerful items and synergies off the bat.
