Get 4 meat cubes in the same run.
To do that, you must kill the 4 Horsemen Famine, Pestilence, War and Death (not Conquest) and get the cube of meat (not the ball of bandages). Best way to get them is finding the Book of Revelations on floor 1 or 2.

Alternatively, if you have Afterbirth, get one cube of meat, get the Box of Friends, go to the Boss Rush, and activate your item many times. You'll get the SMB pretty fast. Also works in Greed mode.

I'd say that Meat Boy is easier to do in Greed Mode - you just restart as Lilith until you have a cube of meat in the shop, then fight the first floor, get your cube of meat, and on the next floor, as long as you don't go out of the room, you'll have your Meat Boy. Ball of bandages, on the other hand, doesn't exist in Greed Mode, so for that you'll have to find a horseman that will drop it and then do the Boss Rush trick.
