Can't Find Vanishing Twin ???

Vanishing Twin is the last item I need for the Dead God achievement. I've been playing for 500+ hours and still haven't run into it ?

I used Death Certificate and looked through every room and still couldn't find it.
I even broke the game in Greed Mode and got everything i could with Chaos until it was only Breakfast, but still nothing?

is my game just bugged ?

It's easy to miss an item in the Death Certificate floor.

It isn't in any Greed mode pool, so farming items there is useless, you'll never get it that way (even Chaos can't help for that).

According to the wiki it can only be found in Cursed rooms and Secret rooms. Farm those with Isaac and his D6 to increase your chances of finding it. Or you could burn a bunch of Eden tokens if you feel lucky, since they can start with any item.

Kill Hush with with Jacob and Esau again. It happened to me. The first time I killed Hush, I died at the same time. Achievement was unlocked but item remained locked.
