Death certificate bugged?

I unlocked Death Certificate by obtaining all completion marks with all characters, tainted and not. After months of playing to get Dead God I got tired and tried multiple seeds to get it (I know, cringe), but other items show up as if it was not unlocked. Any ideas on what the issue is and how to solve it?

death cert. only appears in secret rooms. I reccomend either grinding to get it or crafting mystery present with t. cain and using it in one. It is also very rare, about as rare(if not rarer) than r key. So thats prob why.

Sadly that is not the issue. I grinded a lot, found R key many many times but never death certificate, and as I wrote it's not even present where it is supposed to when using seeds that should have it. I really don't know what is going on

Seed generation changes every time something is added into the game, either from content updates or even mods.
Point is, any seed generated before the latest game update and/or using mods you do not have are unreliable.

If you want to get it reliably, play Tainted Cain. Death Certificate has a fixed recipe: 8 bone hearts.
