what the point of this item
it seem to only have bad effect

Increases your damage stat whenever you decrease another one when you eat pills. Doesn't mean you have to use it with every single pill you see.
So, what's the problem exactly?

It gives damage ups and black hearts for taking negative pills as well as making every pill negative or neutral. Every stat down is also a damage up, and every non stat related negative effect will drop a black heart on the ground.

Situational, its only if you have an abundance of stats other than damage

it's pretty good if you already took some bad pills before finding it - it will give you damage for those on pick-up

It's good if you find dump stat pills like Luck Down, Range Down, or Shot Speed Down. Then you trade that stat for a damage bonus.
Also lets you use other not-so-bad pills (like Retro Vision) for black hearts.

It's surprisingly helpful for survival. You can generate a bunch of black hearts just from this item alone.
