The best way to explain TMTRAINER is that all glitched items you end up picking up are not bound by any rules and can lead to chaos. Actives can work as passives and the other way round, make things happen based on completing rooms or picking up stuff, hitting enemies.. and a bunch of other ways that i haven't mentioned.
Such examples (from my experience) including taking damage every time you complete a room, turning all enemies into hush minions, spawning a drawer on top of you every time you complete a room, give you permanent active item effects like mega mush, small chance of spawning gigabombs when you hit enemies, turn all pickups into crawlers and so on.
thats part of why its so awesome imo
