Did they make any changes to item pools/how rarity works in the past few updates?
My runs have been full of useless junk and stat upgrades. Am I unlucky, or is the game really repetitive now with almost no items to make a run distinct?

There sure were changes here or there but you are just unlucky. They also buffed quite a few items, make sure you don't dismiss them based on outdated knowledge.

Well, some time back (when Rep came out?) they added an "item quality" system to the game instead of the old item weights thing, although I don't know the details of how it operates. I've seen big ticket items like Ipecac and Dr Fetus (twice!) just sitting in treasure rooms during regular old runs in the last month or two, so maybe you're just having bad RNG days. I don't know how often pools get tweaked but they play with the item qualities a bit in most patches, I think.

Item quality applies only to some specific characters/items (T Lost, Sacred Orb etc.), doesn't change anything on normal runs

It was just made a lot better, for example, The Body still has a normal chance to spawn in an angel room, but it was lowered to 0.2 for item rooms, just to make it more of an angel item, but not completely removing it from other pools

I don't remember how this thing was called, but before rep there were some "epic" items. If you got one of them, chances of getting another one were close to 0 (unless you can break the game). That's what got removed in rep

Oh, right, the "special" items system. That's what's gone.

Anyhow the wiki has extensive mechanical discussions (like how weight decrementation works) and is probably more reliable than some nonentity forum poster like me.
