You mostly play like normal as Keeper, but leave a few coins in previous rooms each floor to have healing to run back to. Things like piggy bank and the swallowed penny trinket are godly items on Keeper as well. You can also unlock Keeper starting with Wooden Nickle which can spawn a coin each room

Thanx, i don't have problems on regular rooms but bosses like dogma, hush and mother are tough, also how can i unlock that wooden nickle for keeper?

It is (unfortunately) one of Keeper's unlocks, requiring you to defeat Isaac in the Cathedral

Since you don't have the wooden nickle, try to get your hands on a swallowed penny trinket that you have a chance at finding from blowing up vases.

Go for Cathedral and Hush first of all, those give you wooden nickel and 3 coin health and from that he's a lot more viable.
You may want to save up your coins for devil deals, making sure you have at least 15 or 30 before a boss fight can help you buy deals to help beef him up during runs.
Also compete the Pong challenge, that unlocks you Greed's Gullet, which allows you to gain a lot more health slots for Keeper.
Also battery items are your friend.

Worst-case scenario, his gimmick isn't that game-changing so being good enough at the game can overcome most of his deficits. Delirium will still be a pain in the rear just like it is for anyone who can't obtain significant health, and the second knife piece will still be a pain due to the differences not playing out in his favor.

Two-coin-heart keeper is actually in the long term more vulnerable than the Lost, I feel. Without the third coin heart, you can't use Curse Rooms safely, and if a coin isn't available on the floor and you have one coin heart, you're actually worse off than the Lost in every room until Wooden Nickel decides to cooperate or a coin shows up. That's why it's so important to leave coins on the ground for later use. Of course the situation is different if you have a way of spawning coins even somewhat reliably, and within a given room he's not worse off than the Lost because he has so many ways of getting a third hit (including just starting out with one). But because half-coin-hearts don't exist, he just really isn't durable compared to other characters on earlier floors. The third coin heart is a big deal, and so is the Wooden Nickel. It's a shame that Keeper needs a couple unlocks (and things that are mostly just useful for him specifically--Wooden Nickel doesn't even work differently for him except insofar as it's a source of health instead of just a little extra cash) to not be such a goofy challenge character, but at the same time I guess people want and like the occasional goofy challenge character. "Get good with him" is just sort of how challenge characters work; unlike the Lost, though, his meager advantages just don't make his play style different enough from the regular characters to really make him interesting to play on his own. I sort of wish that you'd get the coin AND the health refill when you use a ground penny to heal, but I guess that takes away some of the incentive to not get hit and would break his interactions with e.g. blood donation machines badly. Wooden nickel offsets that income deficiency to a degree, I guess. Starting with -2 luck just makes him even more dismal, but I suppose it's thematic and also part of the challenge character gimmick.

Anyhow, my solution is just to not play him, because I don't have fun with him. Having to backtrack constantly--especially the end-of-floor backtracking to vacuum up all the coins I left earlier makes runs with him slow and plodding. Sure, you end up doing some cleanup with most characters at times, but that's usually to squeeze some benefit out of extra hearts or something and not just to grab like five cents. Given his abysmal tear rate, I find the biggest problem with him is getting bored and making mistakes. Shooting extra tears makes up for that a little bit with big enemies but not nearly so much with many small ones. I will say that I had a pretty good time with him once when I got Neptunus--of course that's not saying much because it's a good item. I wouldn't encourage you to skip treasure rooms trying for that item, but if you're forced to skip one that might be a nice booby prize.

Since you don't really have the option not to play him since you need unlocks, my suggestions are to get those game-changing unlocks (third heart, wooden nickel) out of the way. I think he plays far better once you have a somewhat reliable source of health plus an extra hit buffer--Curse rooms become usable and Sacrifice rooms aren't as annoying to use, plus you can more safely gamble on Boss Challenge rooms with the chance of regaining a second hit point from the Nickel. Of course that's not very useful advice--I don't actually have any.
