Ultra Hard
After many tries I finally beat Ultra Hard challenge, but no achievement/completion for it?

Check the secrets menu to be sure, number 334.

How did you complete it?
I mean, the challenge specifically requires you to kill Mega Satan by going to the Dark Room. It is however possible to take the Negative and go to the Cathedral, which will end the run earlier and not complete the challenge.

Yup went to the dark room and killed mega satan
