Cant obtain Broken Glass Cannon
I found it as Isaac and got hit by enemies several times while holding it above Isaac's head, but it was not broken after all. What did I do wrong?

Not possible, since this item doesn't exist in AB+.

If you mean the item between Big Chubby and Plan C on AB+, it's the Tonsil familiar, which you can get by triggering the effect of the Tonsil trinket.

Just tested in-game. Works fine. (Both if held above your head or not, since that's not required to break it).
Just to be sure: you do have Repentance installed, right? :dip:

You could try to find an active item and swap your broken glass cannon with it twice.
