Picked up momma mega with the void

I left the void item and then everytime I went to a room it used the Mama Mega Effect anybody know which item could have caused that ?

Mama Mega is a one-time use item. Its effect got triggered as soon as you absorbed it with the Void, so you have it until the end of the floor. Holding the Void or not is irrelevant.
