any mods in afterbirth+ disables achievement unless you killed it lives atleast once

Pretty sure it's until you've killed Mom's Foot once. It's definitely not It Lives but it might be after beating Mom's Heart once.

Yes, that is Mom's Heart. However, as stated before you only need to beat Mom's Foot once to be able to use mods for everything else so killing Mom's Heart for mods isn't necessary.

Yes, i have just bought repentance. And i havent played in a long time, i dont remenber what de items do. I have killed moms hearth in hard.

Hm. Mod support for Repentance isn't back yet, but if it does come back then the requirement to unlock mods will probably the same. Which means that you'll be able to use mods from the get go.
