How much HP UP do you get?

I have noticed that since playing repentance I have gotten an average of +2 red heart per run, sometimes I will get a run where i get more but sometimes I will go an entire run and only receive one heart up.

Did HP up item get decreased occurrence, is this the new normal?

Depends on the run, sometimes, you will get nothing but hp ups and will tears your hair out when you get dmg down or have overall pretty crappy damage, depending on character.

Sometimes you get 3 or 4 Health Upgrades if you are really lucky with the RNG.
