Cannot seem to be able to unlock Jacob & Esau / Strange Door

I've beating Mother twice now, on normal and hard with 2 different characters (Isaac & Azazel) but I didnt get neither the Strange Door unlock or Jacob and Esau. Before you say it, I know the difference between Mom and Mother, I did the route normally from Downpour -> Mines -> Mausoleum -> Corpse. Anyone else experiencing anything like this?

I've also just noticed I never got an achievement for entering corpse....whats happening???

Don't use seeds.
Things done on challenges or Daily runs do not count.
If you enabled the debug console in the config files, disable it. It prevents unlocks (even if you don't use it during the run).
Forget Reruns and Victory Laps. You can only get a few very specific unlocks during VLs, nothing else.
