What to do after beating the mom for the first time?

I just got TBOIR, and beat the mom for the first time. what now?

You unlocked more Levels, with a new "Final Boss". Which will unlock more stuff. Keep going. You barely even started.

Killing Mom for the first time is usually considered as beating the tutorial. Congratulations!

I'd suggest you keep in mind this list. Don't worry if you don't understand all lines perfectly, it'll come with time:
List of things that prevent all kind of progress:
- Seeds (except a few special ones)
- Challenges (except for their reward obviously).
- Daily Runs.
- Reruns and Victory Laps (except for a few specific unlocks).
- (AB+) Any active mod from the mod menu before Mom is killed once on that save (no matter what they do, other saves are irrelevant).
- (Rep) Enabling the debug console in the config files (even if you don't use it).
- Paranoid antivirus programs (or missing rights) preventing the game from creating/editing its save file.

No, just beat mom again, and the run should continue to a new chapter instead of ending.
Generally, the more you play, the more chapters you unlock, making runs longer then earlier ones.

Yep, just keep going on the same file, there's quite a lot to do and discover.
