Dr. Fetus with fast shot speed, triple/quadshot and/or piercing is PAIN, but otherwise it's fairly easy to avoid getting hit after you get used to the weapon. It deals nice damage and is a free passageway to secret rooms. I think it's fine as is.

Yeah, I heavily recommend AGAINST picking up Dr. Fetus if you have high shot speed or piercing as it makes the weapon significantly harder to use effectively. I also obviously recommend not picking those things up if you already have it as I can tell from my own experience that it won't be a fun run. Also, don't pick up Fruit Cake or 3 Dollar Bill with Dr. Fetus either as some of the effects can cause you very easy damage.

He's right tho? It's just 2 little bombs going to the sides. It's not even as bad as something like cricket's body, where's one of the 4 bombs can come in yours direction, still far from the worst combination possible too.

One other thing that should be mentioned is that Ipecac with Dr. Fetus is a fantastic combination as not only do your bombs get a lot stronger, they also make you completely immune to your own Dr. Fetus bombs(just watch out for rocks that break into projectiles and mushrooms as those will still damage you if you explode them). Always pick up Dr. Fetus/Ipecac if you have the former or latter.

Also, in Repentance, explosions pierce boss armor. Though, i'm unsure, wether they've made an exception for Dr. Fetus in particular, since unlike with something like Bob's Brain or Epic Fetus, you can easely increase your rate of fire with it.

It shouldn't because ability to have unlimited bombs is already strong. Bombs and explosion resistance on a non devil deal/angel room item would be quite strong. As you wouldn't really have to worry about your safety and their would be no trade off.
