Sword of damocles should have an effect when its 'primed'
And by that i mean when it has been activated to drop. It should have an effect, like blood dripping from it, or the string should be replaced with a rope and it should be almost cut just barely hanging on by a thread.

Some effect that would show that the sword is ready to fall

I still want to unlock the sword because like suicide king playing card in Co-op its great. As the main negative no longer exists.

Honestly, I think that Damocles should be reworked. Remove the on hit thing. Make it go through invulnerability and lives and make it so that after enough minutes pass(maybe 30 or so?) there's like a 1/400 chance(maybe higher)every second for the sword to abruptly fall onto Isaac.

This makes the item a lot more interesting and it also doesn't make it a direct upgrade for people like The Lost while making it more viable for people that don't die from being stared at. The sword falling on you should be a matter of time, but it could be tricky to balance the amount of time it would take. It would also resolve the issue of not knowing if Damocles is active or not.

this item is good as it is now. you can die anytime so if you don't have any lives you have to take a risk. I had a game with damocles and a dead cat where I intentionally saved my hieropphant and book of revelation charge for the most of the game to be able to live if I die.

This was one of the best runs in a while and I don't want it to be ruined because somebody is salty that he died to fast

Never stated the item was bad, if anything it is a direct upgrade for people like The Lost. I just proposed an idea that affects all characters equally.

I still think that this affects all characters equally. the lost has to pick worse item pool and to be honest I would see this as a kind of reward that you are playing as a challenge character even if you don't have to. besides him everybody have the same affect

besides that I think that the lost can activate it if he dies once and respawn. sword should be active then right? And the whole logic of this "don't get hit and you are free" wasn't it the whole reason the lost exist?
