Also, if you're streaming the game, and someone joins your stream, they wont know if you've been hit or not. Same for skipping in a youtube video.

I suppose a cosmetic effect like that might be flavorful, but would that be true to the story that the item is based on? There was no blood dripping or a fraying thread in the actual Sword of Damocles story, was there? It was always just a sword simply hanging by a thin hair, and that's what the item just is. I don't feel like sprucing it up is really necessary.
Well, I think everything goes from sword of damocles story. If you will know the one, I don't think any visual cues would need there.

Do you know the story behind the sword? Basically the idea is, as King, yes, your word is law, but you could be assassinated at any moment. As a general rule, assassins don't announce their presence.

For the last bloody time, im not asking for a countdown timer to know that i have exactly 3 minutes and 25 seconds left before the sword falls. I want a graphical cue that the sword is in its 'second phase', where it falls. Whereas the first phase is before you take damage.
Pretty sure they know mate, they just disagree with your suggestion, don't get bitter about it.
Also, I thought it did have an indicator? Like it would start to slowly sway around after you took damage? Or was that only in Antibirth?
Nope, you are right. Once you get hit one time, it will start to sway a little more.
Also, the lost or tainted lost with this item = easy win.
