If you keep doing runs you will unlock new stuff eventually. Do every available route with every character. That will unlock some new stuff. As well as just getting through the different levels a certain amount of times.
Its just so demoralizing. Im currently playing the weekly challenge thing and theres SO MUCH new stuff. Like I had no idea that there were other hearts than black and white.
Most stuff is done by just getting all of the endings with every char though. A lot of the arbitrary stuff will just happen along the way. Things like collect X amount of certain items, use X amount of certain pills in a run and so on.
If you just started completely from scratch it can surely be overwhelming to see all the stuff that is hidden behind the late game content.
Don't worry. as long as you enjoy the game. At least there is a lot of stuff left to discover for you.
But if you wanna unlock some end bosses then try doing other things than just Isaac on each character. Maybe play your favorite character and try the other path :)
I've never played Antibirth but I did, at one point, have all the achievements for the base game. Now I'm coming back to play all the new DLC stuff and reading the Wikipedia articles, it is rather overwhelming to see the amount of puzzles and lengthy prerequisites you need to do to unlock other maps and characters.
A lot of this stuff I refuse to believe most people would have discovered on their own without assistance of some type.
Once you have all the endings unlocked, I would focus on unlocking all the characters and getting the unlocks for each character. Also play in hard mode instead of normal, as there are hard mode unlocks for each character. Fill out the win sheet for each character is the main step.
I think I had 50% achievements, which each unlock something new, at 200 hours for scale. It's a long game.
speaking of endings. Are there a cheat sheet for the completed endings for characters? For example defeated Isaac is a cross.

Already got it and knew about most of the bosses. Thanks for the offer though.
