Didn't unlock the lost

So i followed the steps and i died in a sacrifice room while holding the missing poster but i didn't respawn as the lost, instead the run just ended. I don't know if this was because i was playing on hard, if i was playing as Azazel, or if on the final hit from the sacrifice spike it teleported me to an angle room. does anybody know why?

- Challenge runs
- Daily challenges
- Seeded runs
- Victory laps
- Reruns
- Debug console enabled

well if you got teleported then it didn't unlock. Happened to me too.

Yeah i didn't have any of those active as well, it was a fresh new run

You'r playing something newer then Rebirth right ? Because in Rebirth(Without any DLC) the unlock requirement was a different.

Yeah, i have all the DLC including Repentance

You have to die to the spikes, not just in the sacrifice room

this happened to me as well i was playing on Bethany and I died to the spikes while holding the missing poster and I got teleported to the angel room
