I had went into a curse room, picked up an active item. Went into my third room of the game, with enemies. Used said item. The giant gold chest dropped down atop me and I see Steam giving me the achievement of beating it.
Sure enough it's crossed out as well.

Has... anyone else had this happen? Like, what??

Lucky, that challenge is annoying as all hell. I managed to beat it because hearts endlessly duped themselves. (Picking up any heart caused another one to spawn next to me.) No idea how that effect could have happened.

Appearantly, the ending trophy is an entity, that can randomly be spawned by other items. Given the nature of this challenge, this could be intentional. Or - mind blow ahead - this being unintentional is what's intentional.

On my very first attempt I somehow got a combination of items which allowed me to be super-large, so I could walk through everything, and had infinite invincibility. I don't know how or why, because I never changed my Active and then it did that.
The game crashed on the last floor when going through a room. And crashed every single time.

Dear god I know why people hate it now, there were so many attempts, lol

I know I'm reviving a dead thread, but I just had an attempt where those huge, ground-destroying bombs would appear. I never figured out the trigger, but they showed up after defeating Blue Baby and destroyed the trophy. I left the room, went back in, then something triggered the Sword of Damocles effect and I died. Fun times!
