Imagine reading wiki to spoil stuff for yourself and still failing to do it properly.
1. Shovel unlocks the forgotten and you need to do it ONLY ONCE
2. To unlock the door you have to beat the Mother. THE MOTHER. Not "Mom"

Thanks for the correction (I have unlocked The Lost). So when I do all that ♥♥♥♥ with mom's shovel, why has the forgotten not been unlocked?

Spoilerinos ahead.
As already mentioned, what you've done was the unlocking method for The Forgotten.
The Lost has to be unlocked by dying with the Missing Poster trinket held to Sacrifice Room spikes. The Missing Poster trinket in turn is unlocked by beating The Lamb as Isaac.
The Ascend door is unlocked by defeating Mother. It's the new endboss for the alt-paths. The alt-path is unlocked by defeating Hush 3 times, if you haven't done so already. To open the final chapter of the alt-path in Mausoleum II, you also have to find 2 knife-pieces found in Downpour II and Mines II.
After defeating Mother once, a door leading to a special version of Mausoleum will always appear at Depths II. Also, Depths II will now always contain a special marked skull with a "The Fool" card inside.
Little reminder, that doing reruns, victory laps or playing with seeds or the debug console enabled will prevent unlocks.
Victory Laps don't count eh? That explains it. Every time I have gotten Moms shovel and done everything that followed was on a victory lap. Thanks for clarifying. Much appreciated.
Same with dailies and challenge runs. The exception of course, are unlocks, that specifically require challenge runs, dailies or victory laps to be completed.
Just try a more friendlier tone or don't reply at all.
