I really like neptunus + choclate milk combo, so probably this 2, godhead, wafer/cancer and maybe dark bum

Sacred Heart, Technology, Brimstone, Magic Mushroom and Ludovico Technique. Ludovico Technique and mini-Brimstone/Brimstone are a run winning combination that I don't see mentioned too often, and the other items are just there to add damage to make the process even faster.

I just tested it and added soy-milk also, now isaacs mouth is permanently full and it can't shoot. So don't do that :D

Tiny planet+Spoon Bender+ Continuum+Parasite+Lump of Coal. Maybe instead of spoon bender sacred heart for homing plus extra damage. But that combo is really dirty if yall havent tried it.

Not a favourite one (cuz it's hard to say which one definitely is), but Perfection + Ghost Pepper made me feel like Hans "Flammenwerfer Liebhaber"

I haven't actually gotten this one, but just thinking about it makes me laugh evilly. Assuming that the character's starting loadout doesn't count unless it's Eden, imagine this with Tainted Eve:

Gimpy (so you're drowning in extra hearts to turn into clots), Little CHAD (same), Sacred Heart (because base damage can eat a censored and I'll probably never unlock Godhead), BFFs (boosts both), Bumbo (no other items means not much use for coins, although that's always true post-Womb anyway). Bonus points if you can get Mom's Locket as a trinket to turn all those half hearts into full ones.

If I were making it a custom challenge (and crowdsourcing custom challenge ideas is what this sort of topic is for) I'd add the key pieces and make the target Mega Satan, because this loadout melts even him. I'm talking from experience here; even with the damage scaling Repentance granted him, Sacred Heart and the zillions of clots I had by that point were enough that he ended up doing an impression of The Haunt because I was reaching the summon thresholds before he could do anything.

psy fly, psy fly, psy fly, venus/2spooky, and magic mush. pretty much unkillable, magic mush for a speed up and do a little more against noon shooting enemies, psy fly x3 also is beelzebub.

when playing maggie and getting trisagion and lachcryphagy i noticed that that is basically a game winning combo (in that run i managed to somehow instantly kill dogma and all phases after him without doing anything lol)

other than that, dunce cap + tech zero (bonus for almond milk lol)

Replace the Soy Milk with Almond Milk, then throw in Fruit Cake for more chaos and Sacred Heart to make it controlled chaos.

Also i don't unlocked it yet, but Rock Bottom seems to be the most OP item in the game.
