The Beast isn't in a "boss room". I imagine the Beast takes place in a separate arena, something similar to a crawl space (hence the gravity). If so, that'd explain why the Star isn't in the room.
But that still doesn't mean he is in a boss room.
If the room isn't designated as a "boss room", keyword here being "room", then the star doesn't spawn there.
You might be right thinking it's a bug, but I think it's an oversight rather than a bug.
The Dark Room/Chest Interaction, when you have both key pieces makes more sense; It'll simply go to Lamb/Blue Baby. But imo, once defeated, it should start moving towards Mega Stan, if the door is open. It should also move to Boss Rush, if the door is open after beating Mom.
I think it'd be a ♥♥♥♥♥ to code, as you'd have to program the interactions specifically, but it's not a bad idea.
At the least, i think it should spawn in the beast and mega satan fight properly. Also it doesn't work in the Mother fight iirc.
The Star goes straight to the boss room with Delirium in it. If you follow it to that room, you can do all the other boss rooms and then that one for additional items and resources.
It never crashed, at least in my experiences.
