Possible Bug: Save File Image Priority
The save file image is supposed to change to the highest priority achievement that you have gotten, but with Repentance it seems that the "Home" image for beating The Beast has a higher priority than Platinum God.

Well my 1000000% were also changed after the witness. You can always mess a little with the priority but good luck with that
