The new co-op is true multiplayer co-op and is done by joining the game with a second controller (or keyboard if controller is the primary player) BEFORE leaving the first room.
If anyone joins after the first room has been left it will default those players to the old system.
You can have both new and old co-op systems working at the same time.
What is the difference between new and old modes?
the old system is a drop-in drop-out system that allows any player to take hearts from the main player in order to play as a mini baby that has a copy of the main players items.
