Ultra Horsemen magically revived and the fight started over
So I was happily going into the Beast fight like on any other normal run as Tainted Judas, I had a pretty average build aside from getting both Brimstone and Holy Mantle, and the fight was going perfectly fine, took some damage, oh well, I'm just a casual player and such things happen, but that's not the point.

I get to the Ultra Death fight, and it went kinda slow, it almost completed its second cycle of attacks, but I finally defeated it, expecting the Beast to show up as it normally would. Except this time I got Ultra Famine again. All of the Harbingers were back in the background as if the fight never occured and I just showed up. This is the first time something like that happened, the fight just... reset? Although it looked too natural, no cuts in graphics and/or music, it went on as if that was how it was supposed to happen.

Am I just oblivious to some of the mechanics of the fight? Or is this some bug? I can't find any info about how and why this can happen.

D7 effect maybe? D7/D100/D Infinity/Dice bag/Book of virtues wisps with the D7 effect could do that.

Unlike other boss rooms the D7 effect will reset the fight for the beast. Presumably this occurs due to the D7 method to prevent resetting boss rooms being to teleport you out of the room to a different random one and then reset said random room instead - which doesn't work in the beast room.
That said, pretty sure it's not intended.

Hey, that might actually be it. I did not have the D7 or any other dice-related items in my build at all, but what I did have is that one white wisp with a spinning dice inside given by a Soul of Bethany. I can't recall whether I had used the Soul immediately before initiating the fight or sometime during the ascent, I can't even really be sure it actually survived until the fight itself, but that seems to be the only plausible case.

But then again, I clearly remember that the fight restarted after I defeated Ultra Death, and it has no posthumous projectiles that could have extinguished the wisp, so how would it get destroyed in the first place?
