Brimstone never appears
I have a play time of 81 hours and 12 mins. (3ds) and even after beating blue baby. The brimstone never showed. The devil is just a ♥♥♥♥ most times

I've got 540 hours in this game, and I've picked up brimstone maybe 10 times.

Maybe it's rare (if you don't play as Isaac), but stupidly overpowered. Althrough not as rare as Mega Satan's head

Don't pick up "Special" items, they reduce the chance for it to spawn.

*Seeing* them reduces the chance. Whether you pick them up or not doesn't matter.
