Favorite UNDERRATED Items Thread
Here we honor those underappreciated items that no one seems to be talking about, but that have saved your ♥♥♥ on some memorable runs. Maybe they're not as OP as Brimstone, but they're still worth picking up:

1. D1 — I thought this item was trash, until I realized how to use it: any room with a single soul/black heart or health up pill/rune/card can generate MASSIVE amounts of health on any floor. Combine it with the Car Battery for even more hearts. I used it to get the Marbles achievement, and it's probably great for the 10x Tears Up 'cheevo too.

2. No. 2 — Infinite bombs + mass room damage means I will never reroll this, despite occasionally blasting myself with it. Totally worth it for blowing open skulls, blue flames, and secret rooms alone, while the poison gas makes early rooms super easy to clear.

3. BFFS! — Probably not that great of an item, objectively, but I love it especially for how it means more drops from bags & sacks.

Empty Vessel - it's basically free flight and invincibility shield every few seconds. Also a great defensive item for the Lost

Guppy's Head (apart from its contribution to the transformation) -- just...solid
Head of Krampus -- would much rather see lump of coal, but this clears awkward rooms and even kills bosses quickly
Shoop Da Whoop! -- sometimes, it's all you need for that little bit of something extra

Demon baby (the one that auto shot tears) - Has been my favorite since the Original. That small extra help for crowd control in close distances helps and he will make sure every his tear hits the enemy. Independent follower that kill enemies with out Isaac's guidance is always nice.

You will thank him a lot when fighting these annoying zig zag dodging moving spiders.

Golden Remote. Got it twice in my last 5 eden runs. nearly every floor devil deal and error room :D

Lil' Brimstone. He's quite good at early stages
I like distant administration, high damage familiars are great.

Second on Guppy's head — useful early game & late game and so many synergies pump it up. I usually skip the big one-shot boss damage items though... Maybe I need to reconsider them

Another one I completely underestimated... I just now realized it also opens the Mega Satan door if you want it to. Plus stealing Devil Deal items is awesome. Will start favoring this way more heavily

Some other items I just realized are pretty great:

1. Car Battery — I will pretty much always pick this up... Makes so many items go from good to amazing, from heart generation to Guppy's Head to coin drops.

2. The Wiz — I know most people hate this item, but honestly... I think it's really good. Spectral tears are the best, and hitting most enemies from a diagonal rather than head-on keeps your character much safer. Late game, it starts to get insanely good with high damage / shot speed / mutli-shot synergies.

Portable Blood Bank - I used to hate Blood Banks until I started playing The Keeper. If you get this and Swallowed Penny on The Keeper, you have infinite money. It makes the game a cakewalk and lets you buy out every shop. Especially the ones with Restock boxes.

Guillotine - High damaging orbital that blocks shots + DMG Up + Tears Up - heavy upside to a minor downside of having to aim tears as an orbital. It's a 3-in-1 item, at least one of its positive effects will always be useful.

Lil' Monstro is actually really good. His charge time is short and he deals decent amount of damage.

Lil' Harbingers do some good damage with their flies, despite War being terrible.

Blue Baby's Only Friend is basically a weaker Ludo, and can shred early enemies with BFFs.

Speaking of Ludo, too many people hate it. It's an amazing item.

Blood Clot is a great damage up.

How to jump. I know it's not a very good item, but it's fun to use, and I've saved myself with it several times
