The Binding of Isaac is the original flash-based game from 2011.
TBOI: Wrath of the Lamb is an expansion of the 2011 game.
Eternal Edition is a version update which adds some free more difficult optional content.

The Binding of Isaac: Rebirth is the 2014 remake from the ground up with a new engine.
TBOI: Afterbirth is DLC of Rebirth.
TBOI: Afterbirth+ is additional DLC complimenting Afterbirth with more features and mod support.

TBOI: Antibirth is a free fanmade mod that expands base Rebirth (no DLC)

TBOI: Repentance is the final DLC for Rebirth which is an official port of the fanmade Antibirth mod plus extra, final features to cap the game off. Expected to release sometime later this year.

All the original content from the 2011 game and its expansion is in the 2014 remake but with more features, polished gameplay and allsorts. In the end, you should try out the base The Binding of Isaac: Rebirth game to get a balanced sense of the content. The DLC is for people who in general, want more even more content to play with and experience.

Get rebirth, the rest are DLCs (downloadable content, add ons that cost additional money) which you need rebirth already to get them.

I would also recommend not buying it. TBOI 1 and Rebirth were fun. The first few dozen versions of Afterbirth were also fun (Azazel with Monstro's Lung and Rubber Cement for example). Now? It's only the game "creator" pulling a John Romero, and showing you that you are not a real gamer, cuz only real gamers can appriciate his work. And everyone else will be made humble by his genius.

The Keeper? The Lost? THE FORGOTTEN?! Unbelivebly hard to unlock, little to now fun to be played unless you unlock (and later EFFING GET!) specific items, tailored to negate the inherit flaws of the charecters (like The Keeper needing Greed's gullet to even be playable).

Like I said, at it's current state? Not worth it anymore.

P.S About Repentance... they are gonna make up PAY to play something the used to be free? Yup, McM is losing it.

I guess i'm the real gamer, but I believe if Isaac was any more easier it wouldn't be fun. I guess different people are fine with different difficulty, but imo games need to be difficult to be fun, for example I died like 400 times in Cuphead but still enjoyed it greatly.
