Afterbirth also adds more powerful items, though. So it kinda balances out imo.

does it make the game signifcantly harder or just a tad bit more?
Afterbirth+ fixes that and is much more friendly about progression. Feels like a reinvented Rebirth.
yeh. i play the nintendo switch edition with afterbirth + and its pretty great, and im pretty sure they got a load of new items. their bosses are 10/10 too. i mean, wait until you see The Hush! that thing is by far one of the best bosses. but apparently delirium (afterbirth + only) is crap, idk why, because on a good run i can easily kill him and there arn't any sudden deaths, but i hear its bad (technically you are fighting every boss on your run, but their power is amplified and they attack and change faster the more you damage delirium).
over all, id say the afterbirth definitly changed the game, but i really dk, because im the guy who got handed afterbirth + from the start :/ i never tried rebirth, but looking at it, it has a pretty noticable load of stuff missing from afterbirth
it even adds in a new boss for the 3rd gameplay option: greed. (idk if you have it in rebirth, but im pointing out everything that surely will be added if you get the dlc)
it adds in some crazy items too. the Box of friends, apparently doubling your buddies, glass cannon (one shot almost anything or at least do a big amount of damage to bosses but it works vice versa), friendly ball (LITERALY POKEMON but you catch your enemies (not bosses but there is a mod for that XD)), mega blast (GIANT BRIMSTONE but its an item that needs charge, and is obtained pretty hard.) and there are a bunch of items in the dlc + too.
i'd really buy it to upgrade from the normal, but the way i hear it, buying the dlc + isn't much of a biggie, even though with the dlc + you get about double the amount of new items or something along the lines
fk im depressed that the switch version has no debug console...
:/ the closest thing to that is infinite basements. once you get that sorta op item called 'delirious' and infinite battery charges, you will soon have an army of bosses XD
also, in case anyone still ponders to get the dlc, i just realised the delirium boss as well as his most iconic item is just dlc and does not need the dlc +. Delirious (unlocked after beating the boss delirium once) has a long charge like the mega blast, but instead of blasting, it summons a white 'delirious' version of A BOSS. this includes THE HUSH, the game's hardest boss. it includes any boss in the GAME except from delirium itself (i think... though i've tested enough to know he isn't coming). there is even bugged kind of boss spawns for MOM and mega satan, and certainly moms heart.
(true story) one time, and this was the second time i got 'delirious', i was fighting the hush (of course, the game's new hardest boss, as i hear) and i used delirious on the exact stage and ANOTHER HUSH SPAWNED.
do ya know HOW COOL it is to have the game's hardest boss face ITSELF in a one-on-one fight? i didn't get to see the results though... ;-; i accidently left the run, despawning the other hush. but I DID GET A SCREENSHOT AND IT WAS AMAZING :D (srry nintendo switch only so i couldn't get it here ;-;)
