Lazerus: Unsure what he represents

Azazel: A demon version of himself who Isaac blames his sins on

Eden: His 'angelic' self being reborn after suffocating in the chest

The Lost: His soul, not laid to rest and forever 'lost'

The Keeper: A shrivelled corpse of version of himself, other versions were trapped in secret rooms, some hung themselves because they couldn't escape. The 'keeper' of the wealth within secret rooms and shops.

The Forgotton: Forgotten and abandoned in the toy chest by Isaacs mother, after she discovered it. One of the endings implied she sold and destroyed the house, selling the property. Possibly buried his remains, but its never confirmed.
