Tips for Unlocking Butter?

I seem to have hit some kinda bug or something else, as I'm unable to unlock Butter. I already have Rerun, which means that I should have butter unlocked, but it appears that I don't. I've never seen it in any runs, and the Steam Achievement is still locked. Is there a way I can unlock the item and the achievement still? Do I need to start a new save file and unlock both achievements that way? If so, doesn't that mean that I can never get it on my main save? Any tips on how to resolve this are appreciated.

Just do its requirement again, don't wait for it to pop up magically.

Is it possible to unlock it though if I already have the 4th victory lap achievement? Am I stuck not being able to 100% the game again because the trinket won't unlock and thus I can't get it? I've done about 6 victory laps to the third go around and still nothing. I understand why I don't have it as an item, but I fail to understand why I don't have it as an achievement when I have the next one up.

Nothing should be stuck.

Yes, I know, logically the game should have unlocked it by default since you have one with a higher requirement, but it seems it isn't coded that way.

You're supposed to start the 3rd lap to unlock Butter!, meaning kill the Lamb a total of 3 times in a row (the first kill will start the first lap, the original run is lap 0). Make sure you counted right, considering the original run as the first lap is a very common mistake.

As you can see, I'm on the 4th lap now and it still didn't unlock the item.
