d5 on the lost?

hello i noticed in many streams that usually the lost starts with a d4 item, but mine doesn't (i already have unlocked the item). Is that because I have just rebirth? that means i need the dlc to have it at start?

lost also starts with holy mantle but again u need afterbirth to make lost spawn with them

The holy mantle as a starting item must be unlocked though. Installing AB isn't enough, you have to earn it.

Remember that you need to unlock Lost starting with the Holy Mantle first. Meaning you're going to have to complete Greed Mode a few time and Donate around 800~ coins to the Greed machine. (I don't remember the exact number.) Then Lost will start with the Holy Mantle! But the D4 and Spectral Tears buff you'll have from the get go. So good luck!

No, you have to unlock it as a starting item for the Lost. The item itself is still available during runs.
