Can't unlock Telekinesis or any of the "beat chapter while taking no damage" items

Trying to unlock the remaining achivements but for some reason It just won't unlock/trigger. I don't see the items in my stats page and i've cleared the floor 100% not taking damage in a number of ways

1) I got through all the way to chapter 4 with the lost (holy mantle), was thinking ok maybe holy mantle hits count as damage

2) tried as Azezal, got the shovel and skipped an xl floor (thats clearing it no? XD) didn't trigger

3) and my last attempt now 100% made sure I didn't get hit chapter 2 onwards (had mom knife, fights were quick) nothing

I have no mods enabled and I just finished unlocking achivements for the forgotten, no idea whats going on, is there some bug i'm not aware of?

The items that are in the stats page aren't items that you unlocked, are items that you picked up during runs

1) No, Holy Mantle hit doesn't count as damage
