Afterbirth+ fixes, smooths out many of the design flaws of Afterbirth, while being its own beast entirely. It adds slightly less content, but overwhelmingly fixes bugs, tweaks item interactions to be better and adds much more quality of life changes that make it hard to go back to the base game. It has some of the most interesting content and secrets in the game, which is extra fun. I prefer this version.
Surprisingly, jumping into Afterbirth can be a little difficult because it takes some adjustment, but going into Afterbirth+ feels smoother and more polished when it took the time to fix glaring issues with Afterbirth. The two DLC together are definitely worth it.
I've heard a lot of negative things about Afterbirth+. Are these just there from when it was still new and that the issues are mostly fixed by now?

Those reviews were from people who bought the DLC when it first came out (it was quite buggy but it got patched pretty fast), gave up on it, and then never changed their review after all the bugs got fixed...
EDIT: If you look at the dates of the negative reviews you'll see most of them are from around the same time period.
Get rebirth + both DLCs or no balls...
If you're enjoying Rebirth, see yourself playing alot more of it, and can afford the DLCs, go for it.
I'd reccomend getting AB+ as soon as you unlock "the hidden chapter", not going to spoil anything more...
