daily run icons

In the daily runs, on the left under the money/bomb/key icons i often see 2 symbols, a piece of paper with something like DR written on it and a random item icon silouhete near it, what is it? how to activate it?

it's the run destination for the daily run. (Isaac, Satan, The Lamb etc)

The paper does say "DR" which stands for "Daily Run." It's just there to let you know that you're currently playing the daily run. The symbol next to that tells you where the run will end (i.e. which boss you will need to beat to win the daily run). Both are purely informational and not something you can activate. The boss symbols are the same ones you'd find on the completion card for each character. You can see this card at the character selection screen and also when you pause the game. Hope this clarifies. =)
