why do i get the same ending over again?

Because that's supposed to happen, only the Heart has several different endings.

You have to hold the Polaroid if you want to go to the Chest. Otherwise the run ends.

White. The other one is the Negative. It's written in the middle of the screen when you take them by the way. :P

Mom's Heart has 11 endings only

EDIT : Also you should try play another bosses :)

You have to beat mom once, then moms heart 11 times, then isaac and the devil 5 times each~ kill the devil 5 times to unlock the negative which you need to be able to beat the lamb, kill isaac 5 times to unlock the polaroid which you need to be able to beat ??? (Blue baby).
After killing one of them you unlock angel room fights, so when you get an angel room you can bomb it and kill it in a boss fight, do this twice and you unlock dads key, and you get a key familiar following you, after that is there a surprise awaiting you in the chest or in the dark room

I'm sure you already know this site. The description of each ending in your guide are just copy/paste of the wiki, as for all rewards descripitons.

Okay, so I only have to beat Mom's Heart 9 more times..
And that's where the fun really seems to start.. with even more frigging end bosses.

Seeds prevent progress. If you kill the Heart using a seed, it won't count.
