How can a womb be scarred?

Easy, really. Just put a blade inside a womb (or anything to try to abort at home, like a hanger), then "play" a little bit with it. Wait a few days to let the tissue heal, and voila.

I remind you the whole Womb chapter is Isaac's way to reprensent "I wish I was never born", hence why he kills himself as a fetus (It Lives) inside his Mom's womb. A scarred womb isn't really surprising, especially if the abortion attempt is made using rudimentary (non medical) tools.
