The Keeper and Sacrifice Rooms (Afterbirth Only)
TL;DR: The Keeper is really good with sacrifice rooms.

Out of every character in Issac, the Keeper is widely regarded as the most difficult and least rewarding, due to having a maximum health cap of 2 coins, one of which is removed upon a hit, and having a very slow fire rate with his triple shot. However, out of his various flaws shines one of his most potent abilities: his ability to secure a Mega Satan fight very early on. How, you may ask? With Sacrifice Rooms.

In Sacrifice Rooms lie spikes located in the center of the room. If the player is to step onto the spikes, they will take damage (even with flight), but have a chance to spawn various rewards. If the player steps on the spikes nine times, the angel Uriel will spawn and, upon being killed, will drop the first Key Piece. If the spikes are stepped on twice more after that, the angel Gabriel will spawn, and if killed, he will drop the second Key Piece. Each time the spikes are stepped on again grants the player a 50% chance of teleporting to the Dark Room, where they have the option of immediately fighting Mega Satan, or perhaps traversing the Dark Room in order to fight the Lamb. This information is standard for all characters.

The Keeper gains a unique advantage from these rooms that is unavailable to other characters. Coins have a chance of spawning upon the following sacrifices: first (1 coin, 50%), second (1 coin, 50%), fifth (3 coins, 33%), and tenth (30 pennies, 50%). A chest can spawn on the fourth (50%) and sixth (67%) sacrifices, which can drop coins. What this means for the Keeper is that he has the possiblity of getting health from the sacrifices, which can in turn lead to more sacrifices, slowly bringing the player closer to getting each Key Piece. If the player is careful, they can even leave coins on the ground at random places on the floor, giving the Keeper the option of going back for health if coins do not spawn. This allows the Keeper to easily secure a Mega Satan fight even from the very first floor. Of course, the player doesn't need to go to the Dark Room immediately upon getting the Key; they can choose to get the Key and continue normally through the game, trying to get better items for themselves instead of attempting the Mega Satan fight with what could likely be base damage.

All in all, the Keeper may not be a very good character, but he definitely does have some unique advantages over other characters, many of which may not have been discovered yet.

Or just find the Swallowed Penny trinket for infinite(sort of...) sacrifices.

But the keyword there is find it. I'm talking about the Keeper in general natively having a huge advantage over others when it comes to sacrifice rooms.

I just spent the last 100+ runs in a row playing as Keeper, trying to unlock all post-it marks on hard (still got Mega Satan left). Once I went back to playing as Isaac, I really missed some of Keeper's advantages. Most noticeably, I had to actually aim my tears again.
