Random room effects?

So, during my playthrough I've had a number of random events happen where my tears will change to either be going in random directions, or pulsating. I also had a room where a follower was in and followed me around the room. However, it only happened in those rooms. Once I left the room things went back to normal. Is this a bug?

There are a few items and trinkets that have the effects you described, though I'm a tad confused about the familiar/follower. I'll try to list some items that match.

Tear Effects:

Familiars:

Any of those items ring a bell? Do you have any mods installed? Did walking into a room cause the effects, or was it something else, like taking damage or using an item?
