Teleported to Void After beating mom's heart WTF

Is anyone know why i got teleported (the same sound as teleport 2.0 or a teleport card) into the void after beating mom's heart??? i had mega satan's key from sacrifice rooms, equality trinket and quad shot death touch, none of this objects can allow a secret teleport thing ??
It happens to someone else?

Well, actually, you got a very low chance to open a void portal when you kill :
Mom's foot(?), mom's heart, isaac/satan. ( i don't know about ??? and The lamb. )

It happened to me once too, a void was opened at the heart. Didn't take it tho.

You must of been standing ontop of the Void portal when it spawned. Sorry to say.

I saw this by looking into why that ♥♥♥♥ happened. but the thing is i got teleported, i didnt run into the void portal, dont even have the time to move, i just saw a frame the access to hush/bluebaby/thelamb and my angel room and got teleported right away

The portal spawns next to the Cathedral beam when it spawns

mmm ok but in my memory the void portal appears after few seconds?
and when you run into it do you have the sound of using a teleport thing ?

also got teleported to the void without seeing or stepping into a portal, don't know what caused this to happen

do you remember if u had equality trinket ? or both of the keys from the sacrifice room ?

I had none of the two

Same happened with me. Had the teleporting effect, not the jumping in the portal one. It was during my first victory lap.

I can't get RERUN achivement because every time i beat mom on second victory lap, i teleport to the void without portal. I don't know what it means...

Lol this happened to me on the first day AB+ came out. I started a new run because I lost to Delirium :( but when I beat Mom it teleported me to the Void and I got to beat Delirium this time(he was also missing some of his HP already for some reason?). I also got teled to the Void during a run on the Ultra Hard challenge which really pissed me off since you need to beat Mega Satan for that challenge

Did you have the tick trinket? Takes 15% hp off of all bosses.
