His place in the lore is Isaac either kills the fantasy that makes him stay in the chest, or fully embracing it and dying in it.
Or, in other words, "The big scary white teleporting thing that made you ♥♥♥♥ your pants when you fought it."
Well, I would not really call it insanity but like everything suggest: He's in delirium/a delirious state and about to die (ultimately does) because of the lack of oxygen (whoever built that chest, built it too well)
Pretty sure that stuff isint deadly though! So delirium can't be that!(?)...
Delirium is supposed to represent Isaac's life flashing before his eyes as he suffocates himself to death in the chest which is why he only transforms into bosses encountered during that run.
