So.. i just did that achievement with the Lost but I tried it before with Azazel and it somehow didn't work. Could it be that the achievement won't trigger with half a black heart?
Did anyone had this and can confirm maybe?

It has to be constant from start to end of an entire floor and not floor 1 - having more than half a heart at any point during the floor fails it, other than that black hearts should work fine as they're basically just a modifier on top of soul hearts (much like how golden hearts are a modifier of any heart type).

Some people have had it fail with the lost too - I suspect that may be due to picking up soul/black hearts ever so briefly increasing his health above half a soul heart - as it's essentially just constantly reset to half a soul heart. You get a chance every floor with the lost though as you always start with half a soul heart so it's much easier with him still.

It was on second floor, I'm 100% certain, that I never picked up any health and I proceeded to the next floor after beating the boss. (Also I almost beat the caves without picking up health but Big Horn is a ♥♥♥♥♥.)
I thought that maybe the achievement is bugged for black hearts? If that's the case, it could also explain why some didn't get it with the Lost. Since the Lost has half a soul heart, could it be that that will become half a black heart upon taking a black heart?

its not about black hearts, there is something else. I've "did it" twice as azazel, but then olso once as judas an none made it unlock.
did all visible rooms on floor 2 while having half a heart at all times.
maybe you need to clear every room including secrets or even "I am error"??
I already unlocked it on my main save with lost but apparently my connection was down that time and steam didn't get it.

I know this is a super late reply, but I found a solution for anyone who's still looking for it.
By floor, it doesn't mean a floor like "Basement II", it means a floor like "Basement". That being, it wants you to do to both Basements I and II to get the achievement. I just did this as Azazel on Caves.

Who is not reading carefully here? If you start with Azazel, how can you beat the basement starting with half a black heart? The earliest point you can get the achievement is when you beat the caves if you're Azazel.

I had a different interpretation of what he meant. If your interpretation is correct, either the wiki description of the achievement or his statement would be wrong though.

The game wants you to beat both floors in a set, like caves 1 and 2 or depths 1 and 2, to count as an entire floor. The reason this is easier with the lost is probably because he starts with half a heart, and thus is the only character who can do this on basement.

Are you sure about that? The wiki description says something very different.

Pretty sure actually. I couldn't get it to pop when doing multiple runs of basement II as Azazel with one black heart, although when I did caves I and II it popped immidiately. I may do it on the other save files to confirm this.

Very old thread but it just popped up for me after skipping from a sacrifice room to the Dark Room with half a heart. Aka "skipping" floors while on half a heart seems to work.

Probably also works with items that let you go down a floor if you use it immediately after descending to one with half a heart.
